This is a Flat Panel Direct circuit board made by Applied Data Systems, Inc. of Columbia, Maryland, assembly number 620800-10 on bare board PWB 170800-1000 Rev B. It is sold used, as shown in the photos. The silkscreen reads "FLAT PANEL DIRECT P/A", "APPLIED DATA SYSTEMS, INC." and "HTTP://WWW.FLATPANELS.COM".
